RICHARD ALVIN COOK, 62, of Honolulu, died Oct. 23, 1999. Born in London Mills, Ill. An instructor for crew and civilians pursuing graduate degrees aboard Navy ships in the Western Pacific. Survived by sister, Delores Entsminger; nephew, Mark Stephenson. At his request, no service; ashes to be scattered off Diamond Head.

ANN MARIE COOPERMAN, 59, of Imperial Beach, Calif. died Nov. 10, 1999. Born in Honolulu. A special education aide.
